We at Excel Pediatrics feel caring for children is a privilege and we are honored you choose us to be a part of your family’s life.  As parents ourselves, we enjoy helping you nourish your child to grow up healthy and strong.  We also know that choosing a pediatric medical home is one of the most important decisions you can make for your child (and your family).

Excel Pediatrics is comprised of a team of caring physicians, nurse practitioners and support team who are dedicated to our mission to provide our patients with compassionate care, with respect and to make every visit to our office a pleasant experience.  We realize that each child is unique, and as such needs individual attention.

What do I bring to the first visit?

A parent/guardian's photo ID

Your child's insurance card

Medical Records from you previous pediatrician and/or their address/name/phone number 

Any legal guardianship papers (if applicable)

Please print, read and complete the New Patient Forms (under tab: patient forms) and bring them with you to your child's first appointment. This will reduce the time you have to spend in our waiting room and will make for a more pleasant patient experience.
